> Microsoft's MAI-DxO hit 85.5% on 304 NEJM cases with OpenAI's o3, beating 21 physicians who had no textbooks or colleagues. The public can't run it.

Microsoft's diagnostic system is back in the headlines. TechCrunch reported that it beat generalist doctors on hard cases, citing a study in Science.

The numbers: the best setup, MAI-DxO paired with OpenAI's o3, correctly solved 85.5% of the benchmark cases, according to Microsoft. The test, called SD Bench, turns 304 recent NEJM case records into stepwise encounters where a model or a physician asks questions and orders tests. GeekWire says the opponents were 21 experienced physicians from the U.S. and the U.K. WIRED put the headline gap at 80% versus 20%, with virtual costs down 20%. Microsoft says it is more than four times the physicians' rate.

One label to fix: a feed headline calls this system "GPT-5-powered." Microsoft's own description of the top setup names o3.

Then the fine print. Microsoft says its clinicians "worked without access to colleagues, textbooks, or even generative AI." It also says SD Bench and MAI-DxO are research demonstrations, not available publicly. TechCrunch says most of the gain seems to come from the underlying models, not Microsoft's orchestration. Microsoft says MAI-DxO itself lifted every model tested. MIT's David Sontag and Scripps' Eric Topol say the next step is a clinical trial against real doctors treating real patients.

My take: the public can't run this research demo. A score on 304 archived cases, against doctors working with their hands tied, is a demo. Run it on a ward, then call me.
