> A Lancet Digital Health study says Mass General Brigham's FAHR-Face, trained on more than 40 million face photos, can estimate cancer survival risk from one photograph.

A Lancet Digital Health study reports that a model trained on more than 40 million facial photographs can estimate a cancer patient's survival risk from a picture of their face. Mass General Brigham researchers built the system, called FAHR-Face. It has two tools: FAHR-FaceAge, which estimates biological age, and FAHR-FaceSurvival, which predicts survival directly.

According to the paper, FaceSurvival was fine-tuned on 34,363 patient photographs, and the highest-risk quartile had more than triple the mortality of the lowest (adjusted hazard ratio 3.23; p<0.0001). Scientific American reports that in a test group of 3,820 patients, 64 of the 122 with the highest risk score died within 90 days of the photo. FaceAge had a mean absolute error of 5.1 years on public datasets. Among US radiotherapy patients, those who looked five or more years older than their age had a 21% higher risk of death over an average four-year follow-up, according to the Guardian.

The paper says the models generalize across age, sex, race and cancer subgroups. Scientific American notes the survival model was trained on a majority-white Boston-area population, and bioethicist Matthew Liao warns that insurers and employers could misuse it.

My take: a hazard ratio is not a clinical decision. The coverage gives no head-to-head against a clinician simply looking at the patient, and that is the eval I want before anyone calls this a tool rather than a result.
